i played waterpolo on semi prof level until i was 21 and i can tell u, u need super super stamina for that sport. to move fast in water and to do that 4 times 8 minute is just killing you at one point.
but for other sport i would say anything with sking and rugby pretty sure.

must be high level gymnastics, i tried that shit and i wasnt even able to do 5 minute without dying and i can easily play 90 minute football at 30 degrees outside but that shit was killing me
Cross-country skiing. Purely scientifically, they are the most fit people on the planet, based on VO2 max requirement to compete among the elite. Second is probably road cycling.
Boxing is one of them. Not just because of how physically demanding it is and how mentally prepared you have to be, but the politics on the sport itself. Firstly, to be professional, you have to be incredibly fit, not just to be able to make your body do the things you need it to, but also to keep focused. The first thing that starts going when you become fatigued is your ability to think in the ring. Then you have to be prepared to get bashed about the nogging and know that you ARE going to get hurt. Every single move in boxing has an opposite. You have to out think the person opposite you. You have to beat their faints, their counters, their foot movement and then implement your own offence. Then once you have everything you need to become a professional fighter, you still have the issues with making it in the actual business. You have to be handled correctly, have a good team surrounding you. You have to be able to sell fights and if you lose, you may not ever get a break again. Then you may get injured and be out for an entire year and instantly lose all the momentum you gained. The business is brutal and not many can make it.
It depends on what you assume as hardest but i would say Waterpolo. I play waterpolo and its a really complex sport tactic/physic wise.
