Please read till the end, I've tried to summarise it the best way possible.
Around 2 years ago, something happened to my game but I'm not sure exactly when it happened
or what triggered it, I only just realised how bad my game performs when I watch old youtube
clips from my channel. The game appears to run really smoothly and the mouse movement looks just as smooth.

Now, my game feels TERRIBLE. Nothing feels smooth, my mouse movement feels "off" and quite sluggish. Tracking
feels really bad and jittery, making micro adjustments (especially fast ones) is difficult & innacurate.
I get 200-300fps, it's frustrating seeing clips on youtube of people with 150 FPS who have such a smooth game.

You can tell the game doesn't look smooth, and my aim looks bad, extremely static.
I use the same GTX 970 & i7 4790k paired with my 144hz monitor right now as in those clips.
I'm sure the problem is to do with my PC. I've tried several different mousepads & mice before I realised
the problem is with my PC. I've done fresh installs of windows with Windows, BIOS & GPU Drivers.
I've configured CSGO with several commands, tweaked my Nvidia 3D Settings & it helps very slightly.
I've also done lots of WIN10 tweaks such as disabling Xbox game DVR and much more.
This doesn't happen just in CSGO, but it's just more noticeable in CS:GO. My mouse movement on the desktop feels jittery,
like theres some sort of input lag or whatever, it's not smooth. I've been to LAN with my friends & their desktop & game feel
great, I wasn't aware of the seriousness of this issue at the time so didn't take my time to copy anyones settings.
If someone can fix this for me which works for me long term, I will gift someone up to $40 on Steam or something else. I really want to have fun playing the game again.
Before someone says, its 100% not placebo and I can put my life on it.

I've exactly the same problem and I tried almost everything like you.
Can you please write which Windows Version and Build do you have? Mine is the latest 1809 (Build: 17763.253) if you have the same I think it has something to do with Windows Update because I tried everything expect to get back to an older windows update version.
I don't know how I didn't do this before but whatever.
I ran MSI Afterburner to monitor my CPU cores when ingame (I have i7 4790k, 4 cores with hyperthreading so technically 8)
I went into a DM and my CPU 1 was under like 80% load and the rest were less than 10 which is really strange.
Now for the FIX.
Go to the start menu, type msconfig
From here, go to boot -> advanced options
For me, it said 1 underneath processors here.
I ticked it, and selected 8 processors. You should select how many you have.
Apply, you will have to restart your PC for changes to take place.
Hopefully its fixed for you! :)
I bought a new one so it's 100% not the SSD.
Do you have SuperFetch (SysMain) enabled under services? Many say people who has SSD should disable the service and people who has HDD should enable it.
I installed Windows 10 new but this time without login into my Microsoft account which means every setting will be default. I'll let Windows Update install the drivers and test if there is a difference if not I'll go step by step to install the newest drivers and if this also don't work I'll disable Superfetch and some other things and if this all still don't fix my problem then I've sadly to deal with it.
and I forgot to write you the most important thing:
1) Disable fullscreen optimizations for csgo.exe.
Right-click on the csgo.exe file which you can find in steam/steamapps/common/csgo folder> go to Properties.
Check the "Disable fullscreen optimizations" box> hit Apply.
this is the most helpful fix for me so far where I can see a difference.
Another thing which could help you.
Did you check HPET in BIOS and Windows? Make sure you have the same value in Windows and BIOS. For example: Disabled in Windows, Disabled in BIOS would be right but if it's like this: Enabled in Windows, Disabled in BIOS it can cause problems.

1. Click Start button
2. Select Run
3. Type 'regedit' in the open textbox
4. Open the tree 'HKEY_CURRENT_USER', select control panel, then select mouse
5. Right clicking, modify the SmoothMouseXCurve and SmoothMouseYCurve hexidecimal values to the following:

If done correctly, you will notice you are holding a more responsive mouse (if you have been playing with mouse acceleration turned on for a long time, be warned: it does take some getting used to.)
I finally fixed my problem hopefully it will help you too.
I downloaded and installed every latest drivers even Microsoft Visual C++ from 2008-2017.
After this it felt better but it wasn't the real fix for me so something was still missing and then I realized out of nowhere that I didn't upgrade the Firmware of my SSD.
So the real fix for me was to upgrade the Firmware of my SSD.

so if you didn't do both steps do it but if you have still the same problem then you should also try the 1st tweak which helped many people.

Nvidia drivers are to blame, they are shit.
You need to do a clean DDU uninstall, and download 378 or older drivers, not only you will get rid of the stutter, but your fps will increase.
Also be sure to update bios, and use default xmp profiles on RAM in bios settings, ram degrades over time and some timings will not be stable, so you need to increase ram voltage a bit.
Also make sure that you use balanced power mode instead of high performance, and that you remove -high and -thread from launch options.
Make sure that your overclock is stable with 30 mins of prime 95, make sure that you have enough voltage, or lower the clocks if temps are too high in prime95.
Try to swap ram slots as well.
Good luck, troubleshooting is a pain in the ass sometimes, but mostly people have really bad overclocks/cooling or they have no idea how to optimize the game... On the other hand cs runs as crap since panorma update on many rigs...
I have 2 rigs, one with i7 7700+gtx 1070tiwhich has super high fps but stutters, and Ryzen 7 2700x + vega 56 which has less fps, but it's smooth as butter in all games, flawless experience.
No, about windows power management, because high perf might come with stutter because of the bad overclocks or unstable temperatures cuz of bad cooling

What was the solution?
For some reason, only 1 core out of my 8 was being put to use
I monitored the game with MSI Afterburner and core 1 was being used like 70-80% and the rest beneath 10, so I looked it up online for a fix. im just gonna copy paste what I said to others below

If you're prepared to reinstall your os you should be prepared to install a better one
